# AI risk management and mitigation evidence

For Health AI, you are expected to show **how you identify risks early and what you do when something goes wrong**.

#### What you need to show

* What could realistically go wrong with your AI system.
* Which risks matter most at your current stage.
* What technical or organisational measures are in place to address them.
* How you know whether those measures are working.

#### How OPORA Core supports this

OPORA Core includes:

* Structured workflows to identify and categorise AI, data, and regulatory risks.
* Clear links between risks and the controls that address them.
* Ongoing monitoring of risk indicators rather than one-off assessments.
* A record of risk reviews and mitigation actions over time.

This allows you to show that risk management is part of daily operations.
